Where will the cats be trapped and returned?
At this address, are you:
Are there any kittens?
Note: Do NOT move or touch kittens. Kittens need to be with their mothers until 6-8 weeks of age. Separating kittens from their mother can cause their death in as little as a few hours, depending on age. Disturbing a site where a mom has kittens will cause her to move them, making it more difficult to rescue them.
Are any of the cats friendly?
Are any of the cats sick or injured?
Under MA law, a wounded cat with unknown vaccinations must be quarantined for 4 months or be euthanized. Are you able to care for a quarantined cat for 4 months?
Do you need to borrow traps?
Your participation in trapping and transporting is required. If you need assistance with these tasks, please provide the reason:
I agree to the following:

1. I have authority to give Carmah and its volunteers permission to trap, neuter, and return cats at the address I provided.

2. Each cat will be vaccinated for rabies and distemper, left-ear tipped, and spayed/neutered.

3. I release Carmah and its volunteers from any and all liability arising from damages to person(s) or property.

4. I understand that Carmah does not take responsibility for any unforeseen mishap that may occur to a cat while in its care.

5. I understand that the TNR program is intended for stray cats and not personal pets.

6. I understand that TNR means the cats are returned to the site where they were trapped.

7. I give Carmah permission to remove friendly cats from the address provided and adopt them to new owners.
